Hi, I’m carolyn.

I am an Acute Care certified Nurse Practitioner with over 12 years of experience in the intensive care unit (ICU) and I absolutely love what I do. In addition to providing care for patients across various medical and surgical settings, I also help individuals among all age groups navigate inpatient and outpatient healthcare services. Throughout my career, I have supported countless patients and their family members in navigating difficult end-of-life decisions during what may be the most difficult time in their lives.

These experiences are what led me to establish Clarity.

Too often, I have found that patients don't always fully understand their diagnosis or treatment plan, or may not have determined and documented their wishes prior to a life-changing medical event. Through this venture, I intend to support and guide individuals and their families during their medical decision-making processes, ultimately providing clarity on their options. Whether an individual is healthy and active, suffering from an acute critical illness, or approaching the later stages of chronic disease, creating an Advance Care Plan and understanding one's treatment options is of paramount importance to ensure that one’s healthcare wishes are honored.

My Approach

Initiating conversations regarding illness or advance care planning can be quite scary and overwhelming—even for those of us without any ailments. However, having these discussions prior to an unexpected diagnosis or hospitalization can ensure your wishes will be executed.

My experience managing chronic and acute conditions affords me the ability to guide my clients through potential hurdles and “what-if” scenarios during the preparation of their advance care plan. I will also prepare the individual and family for circumstances they may encounter during hospitalization, and will discuss the need for support services which may be needed during and following hospitalization.
